05/06 UD Ice - Frozen Fabrics Patch - Brendan Shanahan (BV $50, 2-Color: White and Red).

Have to add that the threads in the patch are a little loose and frayed, but I assure you, I just pulled it today (bought 3 boxes off eBay hoping to strike it rich... not so lucky...).
