Try this and make changes to fit your needs...

    Dear Mr. Oriole,

I am a fan of yours etc. ect. (for example)I remember when you made this great play etc. etc. I was wondering if you could take time out of your busy schedule to sign my xxxx. I would really appreciate it if you would.

    I wish you the best,

    Xxxxxx

This has worked for me since I think short and sweet is the best way to go.

    Good luck!

I pretty much do the same letter every time unless it is a really personal autograph request.

Hello Mr/Mrs dfjsdlkf my name is Tony and I am a huge fan of yours. (Something about their career). I was wondering if you could please sign my ...(cards, photo, ball, etc.). It would really mean alot to me.

    Thank you
    Tony

    Handwritten always. Seems more personal.

Baseball cards if you have them is the best. I've seen 8x10 photos and ROMLB's used for the players worth the cost of those items.
