Michael Jordan All-Star Weekend Unique Card Question

I have had this Michael Jordan All-Star Weekend card (pictured below) for many years since my childhood. I have been looking it up and I have not been able to find a single variant online that looks like mine. Has anyone else seen this particular version of it that has the perforated edges? I am planning on getting the card graded but I have no idea of its worth or where it came from, since I haven't been able to find another like it.

Yes, it's from the 1990-91 Hoops Team Night sheets, which I believe were a giveaway at games.
