Can someone please refresh my memory as to how the playoffs are formatted?

8 (of 12) teams make the playoffs.

Then it's just a weekly head-to-head matchup. 1 vs 8, 2 vs 7, etc.

There's 22 weeks in the "schedule". I'm not sure exactly when week 23 is (sometime in early March, roughly?).

Can someone please refresh my memory as to how the playoffs are formatted?
