You can go to Beckett and Psa both and look at their forms:
http://www.beckett.com/grading/
http://www.psacard.com/services/card..._tickets.chtml
I hope that helps as a normal card will run ya about 10 dollars each unless submitting in bulk. Have any questions feel free to ask and I will try to help you or perhaps other will chime in.

My recommendation - since you're in Canada - is to seek out an LCS or a Canadian distributor that deals with BGS or PSA. Here in the Toronto area and in Montreal, some shops deal with Grosnor distribution who deals directly with BGS. My LCS charges me $25cad per card plus $10cad insurance for every 1000bv. It sounds expensive, but the primary reason I do it this way is to avoid paying customs fees when the card gets shipped back to me (if I were to deal with BGS directly). BGS does offer both insured and non-insured return shipping, but when you're sending a high-end card (or a bunch of high-end cards), I think you'd want it insured. It is also very convenient. All I do is drop off the card(s) to my LCS, fill out the form, and wait until I get the call. The only downside is that sometimes Grosnor takes a while to ship the cards. I waited as much as 2 months a couple of times this year. Hope this helps.
PS: If you can't find one where you are, contact max91ess and see if you can reach some sort of an agreement.
