    Intrument to determine Mental Health

    Does anyone know an instrument fairly simple (survey, questionnaire, etc) which is used to determine mental health? I know I have not been around lately, and have a lot of cards to send to people for the giveaways when I ever get home! I am writing my Masters Thesis due tomorrow, 30 pages in, and realizing I do not have a good instrument to attach for determining mental health, which is written up as the focus of the entire study. Please let me know if you know any, it would be greatly greatly appreciated. Thanks
