LeBron experts: Question about a card

I don't know an awful lot about basketball cards but came across this card in a box that has been sitting in my basement for a couple years on a shelf. I have no clue where it came from or what it is. My bet is an unlicensed issue but I figured I would get the opinion of the experts before I do anything with it. Can anybody shed light on this?

    Thanks in advance, Adam

just that its one card out of 500 made, its not numbered 1/500
thats how they used to do it in the US in the early 90's
