I have pulled a few hot boxes through out the years. Busting cases is how I come accross the hot boxes. I do not know if they are intentional or not but I do know back in 94-95, Fleers basketball products were loaded with hot packs, not hot boxes. I oppened 2 cases of this product in 1996 and I had quite a few hot packs, all inserts.
As far as newer products I havnt sen it as much, I am guessing a Hot box these days are when you get 3-4 G/Ued when you usually only get 2 per box

Donruss tends to do hot boxes every now and then for some products....

From what I've found, it happened mainly with baseball products, like last years Diamond Kings Update, Studio, and Throwback Threads..

I personally bought a hot box of Throwback Threads from my local hobby shop and pulled :
    2 RC Autos
    1 Dual Patch/Auto Tommy John
    4 Triple Jersey Timelines cards
The other 17 were various GU Bat/Jersey cards numbered under 150...
