Normally three paragraphs...introduction, about them, and asking for the autograph with respect. Always thank them and I usually sign the letter but not with my actual everyday sig...

This is what I wrote to Andruw Jones, going in the mail this week...
Dear Mr. Jones,
You were one of my favorite baseball players. I grew up a fan of the Indians but my dad always liked the Braves dating back to their years in Milwaukee so I always closely followed them as well. And since you and I have almost the same first name and we both share a birthday of April 23, it was only natural that I would follow your career.
Enclosed are a few cards I'm hoping you could sign for me. I collect autographs and would love to add some from you to my collection.
