what exactly do I do just, it a certain size envelope, what exactly do I on the index card.

You need a Number 10 envelope. Then you just fill out your name and address on a plain white index card, and mail them to the addresses given here. Good luck Ryan!

quick question..... what if you dont use a 10 envelope... do they just keep them... or do you know why it needs to be them??
