Got one!
This one came up on eBay recently, now I have it:
'98 SP Authentic Sign of the Times Level 1 #S2 autograph redemption card.
Ted lost this ride halfway through the year that year, so he refused to sign the cards for Upper Deck, (I guess they didn't get them to him before then) so the redemption card is the only way this card was available. Subtract all of the ones that got redeemed (UD honored them with other cards from the set) and the population of this card is quite low.

Thanks to Scott, I can now cross this one off the list:
1995 Upper Deck Electric Gold #240
Want list is now down to 20 to go, and the have list is up to a sort-of-even 950!

Ill look this weekend when I'm at my dads. We have to have Musgrave cards somewhere, they are on the older side (which is what we have most of). I'll get back to you.

Just picked this one up in a lot on eBay:
1996 Ultra Atuograph Redemption - no number
Now if I could just get my hands on one of those '96 Flair redemption #11 cards, I'd be done wth autograph redemptions!
