It's a lot more frequent than we think. It's even better when the picture is of the wrong player, though. lol
Case in point:
Justin Falk 2010-11 Limited Phenoms

Drew Bagnall 2011-12 Victory

The player features is Drew Bagnall, so the Phenoms card has the wrong player picture.

Wow now thats pathetic, that is just completely rediculous they should be ashamed, I wonder did Faulk notice when he was signing these lol he had to of, and yet Panini said "ahh whatever its too late now"

He probably had no clue, as I believe Panini would've just had him sign the swatches and they assembled the cards later on, so it's all on them, I think...

You're right - they had the players sign the swatches. Once they were received they assembled the cards, so chances are he never saw the final product and to this day probably doesn't even know. It's a fail, but a funny one.
