I send letter, SASE, and depending on how many cards I'm sending. 1 card, I'll send along 2 index, if it is more than 1 card then I just send 1 index. Really no purpose in it, besides maybe another thing they'll sign and send back. Really doesn't protect the card much.
Never have I sent in a sleeve or toploader. I'm not about to waste my time getting back a signed toploader or sleeve.

Toploaders often get jammed in the postal machines. That's why they bend the envelope to check if it's flexible. If it isn't they send it under a different class to be handsorted. Also as mentioned if a player dosen't want to be bothered or is in a huge rush they just sign the sleeve/toploader.
I use a piece of thin cardboard. Cereal boxes and posterboard is perfect. I just cut them to size and write on them "Can you please put this in the return envelope to protect the card". Or when I use the smaller 3x6 envelopes I cut the cardboard to size and put it inside the SASE. It has been working well for me since 2003.
The only other way I can recommend is using bubblemailers. Obviously it is much more expensive; but if you send more than 5 cards, or it's a card you really don't want to risk getting damaged just use the small snap cases (20 count I think) inside the bubblemailer. If you do this it also gives you the opportunity to include a blue Sharpie.
