I need help identifying the "card" in the photo.
It is etched glass and has a 2001 date and a 1/2004 etched. any other info would be appreciated.

The card is a 2001 Verigraph Brett Favre Etched Crystal Card and like you said, there are 2004 Favre cards though they are not serial numbered and they all say "1 of 2004". I was able to come up with a couple of names also on the checklist and from my understanding most of the cards were limited to 2000+ the players jersey number. I was not able to find any Favre cards on eBay for sale or recent sales but there are some of the other players available along with some other cards from different years.
I read that there are 13 players on the 2001 checklist but here are the players and print runs that I could find:
Jerome Bettis- unknown copies total
Brett Favre- 2004 copies
Doug Flutie- unknown copies total
Peyton Manning- unknown copies total
Donovan McNabb- 75 copies
Emmitt Smith- 2022 copies
LaDainian Tomlinson- 200 copies
Michael Vick- 500 copies
Kurt Warner- unknown copies total
This is what the original packaging looked like:

Thank you!
I have about 5000 different Favres and hadn't seen this before and couldn't find it on my checklist. I appreciate your efforts!

I am a fan of oddball cards but it doesn't appear that there are any Seahawks on the checklist so I am out of luck. Is the card heavy duty or is it a delicate type of crystal?

it's fairly thick. About 1/4 to 3/8 thick. When I compared the Facsimile auto it is Identical to an Authentic image 1998 gold card Auto though, so I assume Verigragh was made by Authentic images
