Got an error adding an address, and now there's multiple submissions of the same address. Can the duplicates be deleted?

If you had issue in the past hour with submission database error, my bad. I have been recoding the whole player stats function and made a small mistake.
Mainly the last success date was not done correctly and now I know it fine and should not have to touch the submission again.
I just added a new column to the top 25 player's signing. It's the last success date. Also you can query the top 25 players for the last 3, 6 months and last year also!
I also just added more stats to the player's list on a second line to leave the player's name more visible and also made change to the last activity by changing the number of days color coding making it one week, one month, three months and a year as a new pink color. Si it goes as follow:
Below the player name you have statistics. First the player's latest activity, it will have (or not if more then 365 days) a colored number with stars.
Stars Legend: Green star(*5*) between 0-30 days, Gold star(*28*) between 31-90 days, red star(*78*) between 91-182 days, pink star(*324*) between 183-365 days
Also, if the player as more then 0 percent signing, we show the percentage and number of successes. No text on the second line mean the player his not active at all.
So if you see *78* 100%(4) it mean the last success was reported 78 days ago with a success percentage of 100% based on 4 success and being 100% it mean 0 negative.
Let me know what you think of these innovation and if this will be enough. I don't want to overcrowd the player list and this is why I moved to stats on a second line to make it easier to see the player's name.

I deleted the extra addresses. Not sure why it happened. But I will do some more test with long instruction to make sure it can't happen again.

Just seen the new format. I'm having problems. I had 2 successes today in football. When I search their names no one comes up (Paul Moyer and Ryan Lindley). When I click on "My TTM report" I'm only seeing things for Hockey. Most likely I'm just stupid and can't figure it out so any help would be appreciated. Thanks

Currently the TTM is close until I fix a table that is corrupted. Not sure how long it will take as we are online with our host so they can provide us with a database back up so I can fix that one table. Once I fix the issue it should be hockey, but to be sure I will PM you so we know the issue is fix on your side as well. Sorry for the inconvenient.

Looking good!
I'm getting an error when I try to add a new address.

This was the error I saw when I tried adding an address last night that turned it into duplicate entries.
