not sure if these classify as "worst", but 2 that stick out in my mind are from the diamond:
Jason Kendall's compound fracture of the leg. Bone went right through the skin and his pants. he said that the stirrup sock was about the only thing that kept his foot on.
Ruben Mateo: blew out his knee and i think broke his leg running to first. when he stepped on the bag, his leg just blew up, and on the follow thru, it looked like he kicked himself in the back of the head.
And i'd also have to say that the McCallum leg explosion is one of the worst.

Don't forget about Darryl Stingley, who just recently passed away. He was paralyzed by a hit he took from Jack Tatum back like in 1976 I think.

I think the worst one I remember was at the end of the 1968 World Series. When the Cardinals lost, it broke my heart.

Not the worst but Prothrow will never play football again after that bad broken leg in llike the third game of Alabamas 2005 season. Amazing catch though
